Student Conduct Agreement *

I agree to the Student Conduct Agreement

By registering for a Paratus Bellator Group course, the parent/guardian and student acknowledge and agree to the following:

Students are expected to behave respectfully toward instructors, staff, and fellow students at all times. Disruptive behavior, bullying, or disrespect toward others will not be tolerated.

Students must use appropriate language during all classes and activities. Profanity, offensive language, or inappropriate remarks are not permitted.

Students are required to follow all instructor directions and safety guidelines during the course.

Paratus Bellator Group reserves the right to dismiss any student from a class for disruptive behavior, unsafe conduct, or failure to follow these rules.

If a student is dismissed from a class for violating these rules, the student will not be permitted to continue and all course fees will be forfeited. No refunds will be issued.

By checking this box, I confirm that I have read and agree to the Student Conduct Agreement and understand that failure to follow these rules may result in dismissal from the course without refund.

Assumption of Risk & Hold Harmless Agreement (copy) *

I, the undersigned parent or legal guardian of the above-named student, understand that participation in the Essentials of Wilderness Survival Series involves outdoor activities that may include, but are not limited to:

• Fire building
• Use of edged tools
• Shelter construction
• Land navigation
• Primitive skills
• Outdoor movement over uneven terrain
• Exposure to natural environmental conditions

I acknowledge that these activities carry inherent risks including injury, illness, property damage, or other unforeseen events.

I voluntarily assume all risks associated with my child’s participation.

I agree to release, indemnify, and hold harmless Paratus Bellator Group, LLC, its owners, instructors, volunteers, agents, and affiliates from any and all claims, liabilities, damages, or expenses arising out of or related to participation in this course.

I certify that my child is physically able to participate and that I have disclosed any relevant medical conditions.

I understand that Paratus Bellator Group maintains safety standards and instruction protocols; however, accidents may still occur.

By accepting this agreement, I acknowledge that I have read and fully understand this waiver and voluntarily agree to its terms.

Dear Parents, Thank you for registering with Paratus Bellator Group. If you have chosen to use Step Up for Students funding—and we are a Direct Provider—please log in to your EMA account and search for PB Group in the provider section. Once you locate our listing, scroll down through the available courses. Under our EMA listing, look approximately three-quarters of the way down for Essentials of Wilderness Survival with a fee of $75, and proceed to reserve your funds. This must be done for each child registered. If you are registering for other classes, please be sure to match the correct listing with your selected program. After funds are reserved, you will receive an invoice via email that will only allow for payment by Gift Card, that is how we reserve the funds. You will receive a receipt when we pull the funds.
